Holistic Primary Care LLC ("we," "us," or "our") runs the website holisticpc.com. This policy explains what information the website collects, how we use it, and the choices you have.
Information you give us
Contact form. When you use the contact form, we collect your name, email, phone number, the reason you're reaching out, and your message. We use this only to respond to you. Please don't include medical details in the contact form. If your message involves your health, call us at (727) 688-2235 instead.
New patient intake forms. Our intake forms are handled by MailHippo, a separate HIPAA-compliant forms provider. When you open the intake form, you leave our website and use MailHippo's secure system. The health information you submit there is protected under HIPAA and handled the same way as the rest of your patient records.
Information collected automatically
Google Analytics. We use Google Analytics to understand how people use our website, like which pages get visited, how long people stay, and what type of device they use. Google Analytics uses cookies to collect this. The information is grouped together and doesn't tell us who you are. We use it only to improve the website.
You can learn how Google uses this data at policies.google.com/technologies/partner-sites. You can opt out of Google Analytics on any website by installing Google's opt-out browser add-on.
YouTube videos. Some of our pages include videos from YouTube. When a page with a video loads, YouTube (owned by Google) may set cookies and collect information about your visit, even if you don't play the video. This is controlled by YouTube, not us. How we protect your information
Our website uses an SSL certificate, which encrypts information sent between your browser and our site, including anything you submit through the contact form. Contact form messages are delivered to our office email.
What we don't do
We don't sell, rent, or trade your information. We don't send newsletters or marketing emails, and we don't add you to any mailing list. We only share information when it's needed to run the website (like the services listed above) or when the law requires it.
Links to other websites
Our site links to other websites, like Facebook and Google Maps. Once you leave our site, their privacy policies apply, not ours.
Your choices
You can set your browser to block or delete cookies. The site will still work, though some features like embedded videos may not. If you'd like us to delete a message you sent through the contact form, just ask.
Children
Our website isn't meant for children under 13, and we don't knowingly collect information from them online.
